Form Requests and Automation

What are Form Requests?

A form request is an invitation sent to a specific person (or group of people) asking them to fill out a particular form. Instead of sharing a generic form link, form requests let you target specific individuals and track who has and hasn't completed the form.

Creating a Form Request

To send a form request:

  1. Navigate to the Forms section
  2. Click Send Request next to the form you want to send, or click New Request in the Form Requests panel
  3. If starting from New Request, select which form to use
  4. Choose the recipient(s) — you can select individual profiles or use a smart list to target a group
  5. Configure any additional settings (message, due date, reminders)
  6. Send the request

Tracking Requests

The Form Requests section shows all active requests with their completion status. You can see:

  • Incomplete requests — Requests that haven't been submitted yet
  • Completed requests — Requests where the form has been filled out
  • Request details — When the request was sent, to whom, and the current status

Reminders

For requests that haven't been completed, you can configure reminders. Reminders automatically notify the recipient that they have a pending form to fill out. This helps improve completion rates without requiring manual follow-up.

Form Automation

Forms support several types of automation that trigger when a submission is received:

Auto-Create Profiles

When a form submission comes in, Qik can automatically create a profile record using the data from the Person field group. If a matching profile already exists (based on email or name), the submission can be linked to the existing profile instead of creating a duplicate.

Notification Automation

Configure automatic notifications to alert your team when submissions arrive:

  • Email notifications — Send an email to designated recipients when a form is submitted
  • In-app notifications — Display a notification in the admin dashboard

Workflow Integration

Submissions can be automatically added to workflows for further processing. For example, a volunteer application form could automatically create a card in your 'Volunteer Onboarding' workflow, routing the applicant through your review process.

Managing Active Requests

From the Form Requests list you can:

  • View all pending and completed requests
  • Resend requests to people who haven't completed them
  • Cancel outstanding requests
  • Review submitted responses

Tips for Effective Form Requests

  • Set clear due dates — Give people a deadline so they know when to complete the form
  • Use reminders — Automated reminders significantly improve completion rates
  • Personalise the message — Include context about why they're receiving the request
  • Track completion — Regularly check the Form Requests panel to follow up with non-responders
